Chinite Homecare is looking to recruit a full time, driven and self-motivated Care Coordinator to work within our busy office in Barking to run service provision in Essex.

The successful applicant will be responsible for the ongoing coordination of domiciliary care workers and will ensure that service user requirements are met whilst maintaining compliance of care standards in accordance with Local Authority policies and procedures and CQC regulations.

You will be required to provide day-to-day support to care workers and service users assisting with the assessment, set up and review of health and social care packages, and you will be required to ensure that a skilled workforce is in place, and they are able to deliver safe quality care to all clients.

Duties include:

Ensuring that the client receives the appropriate care package, and that continuity of care is a priority.
Providing support to clients and care staff and where necessary visit site to resolve matters
Accept, allocate and process new care packages.
Produce rota, monitor care visit using our inhouse monitoring system
Undertake quality assurance checks or monitoring of care packages
To assist in auditing the standard of service delivery to clients.
Attending review meetings with client / family / local authority / health personnel as require
To supervise carers in the field and office staff
Participate in the recruitment and training of care staff.
Any other administrative procedures required by the Director / Line Manager
Work unsocial hours as per the needs of the packages.
Demonstrate the ability to manage the caseload effectively, dealing with unexpected issues e.g. sickness, missed calls
Provide scheduled KPI to management.
To participate in on call duty rota

Requirement:

a. You must possess NVQ Level 3 in Adult Care or above

b. Knowledge of rostering and electronic monitoring system desirable, however not essential (we work on Staff Plan).

c. Clean Drivers licence and access to own car but Company pool cars will be available too.

d. Basic salary+ on-call Fee + mileage/expense reimbursement
