The starting salary for this role is £27,991 per annum for a 36 hour working week.

We are looking for a tenacious and creative team member to join our expanding Communities and Prevention team within Adult Social Care. With ambitions to reduce health inequality, promote independence and achieve great outcomes for people, the successful candidate will be experienced in supporting people with mental health conditions and passionate about community inclusion.

In Surrey, our ambition is to embed a model of social care that grows local resources and encourages behaviours which benefit the individual and enables the council to deliver services within the resources available. The role will create an environment whereby individuals can be diverted towards informal, community-based (Mental Health specific or other suitable options) services that prevent, reduce or delay care and support needs from developing, as required by the Care Act 2014.

The successful candidate will build excellent working relationships with internal partners and external providers, and work alongside a range of professionals and volunteers. They will work collaboratively across the system, including with frontline social care teams, Public Health, Surrey and Borders Partnership Trust and voluntary organisations.

The main focus of the role will be to identify, promote and develop community-based support for people with diagnosed mental health conditions supported by adult social care, so that they stay healthy and independent in their own communities.

The successful candidate will share information and resources, support practitioners to take a strength-based approach, and undertake community development and other projects to create new forms of support. They will work with voluntary and community groups to build inclusive and self-sustaining models of support, which reduce stigma, promote independence, tackle health inequality and achieve better outcomes for people with mental health support needs.

Key outputs include:

  • To ensure a wide range of information regarding local voluntary sector services, support networks and solutions are available and accessible to mental health social care teams as well as hospitals and other health care settings
  • To identify gaps in provision of mental health support and work with local partners to develop a range of community assets, upon which residents and communities can draw on to improve wellbeing and increase independence and resilience.

Though part of a team of Communities and Prevention Coordinators, the post-holder will have sole responsibility for mental health within the team. The postholder will work proactively to co-design and deliver initiatives that support Surrey’s 2030 Vision within the context of Adult Social care, showing innovation and perseverance to bring about change.

The ideal candidate will need to have a good understanding of the needs of mental health service users. Knowledge and experience of working with adults with a mental health condition, or people with different social care needs in a health or social care setting; in paid or voluntary capacity is also a benefit.

The successful candidate will be required to travel around the County to meet the community and partners in various locations. In line with SCC policy, efforts will be made to reduce the travel required where possible. Flexible working will be supported through mobile technology.
