The role:
The OPPD Service supports older adults and adults with a physical disability. We are a multi-professional service comprising of Social Workers, Occupational Therapists, and Assistant Practitioners. We are recruiting to an Assistant Practitioner post within our Assessment and Intake Team.
The Assessment & Intake Team provides an initial assessment function. We provide a multidisciplinary approach to assessing and supporting people and work using a strength-based approach focusing on people's strengths, assets and abilities. This enables us to support individuals to make greater use of their community resources and to maximise their independence.
As part of the team you will be responsible for completing Care Act 2014 assessments to determine whether or not an individual is eligible for services. This includes the undertaking of proportionate assessments, assessing ongoing support, support planning and proactively working with complex issues, family dynamics, legal issues, crisis, providing information and advice, transferring to another team if required and the provision of some ongoing safeguarding work, under direct supervision from a qualified professional.
You will also be asked to complete Carers assessments to ensure that we are supporting the numerous informal carers within the borough.

Within this role, you will work with persons with care and support needs and other professionals as necessary in order to meet the needs of the Service.
