Description

The starting salary for this role is £24,780 per annum based on a 36 hour working week (pro-rata for part-time staff).

We are looking to recruit Social Care Assistants within our Adult Social Care teams in North West Surrey.

We currently have vacancies in the following teams:

  • 2 x full-time positions based at Runnymede Locality Team, Runnymede Civic Centre, Station Road, Addlestone, KT15 2AH. Part-time applications will be considered.
  • 1 x full-time position based at Woking Locality Team, Woking Borough Council Civic Offices, Gloucester Square, Woking, GU21 6YL. Part-time applications will be considered.
  • 1 x full-time, fixed-term/secondment contract to cover maternity leave up to 1st March 2023 and 1 x full-time, fixed-term/secondment contract up to 21st March 2023 based at Spelthorne Locality Team, Ashford Centre, Stanwell Road, Ashford, Middlesex, TW15 3DU.
  • 1 x full-time position based at St. Peter’s Hospital, Guildford Road, Chertsey, KT16 0PZ. This service runs from 8am to 8pm Monday to Friday and 9am to 5pm Saturday and Sunday. In practice, you are likely to be required to work a minimum of one weekend shift per month and one late shift per week. Staff who are on the rota to work a shift falling on a weekend will be given a day off in lieu. Full-time applicants will be contracted to a 36 hour week and these hours will be worked on a rota basis across 7 days a week. Further information regarding the working pattern can be discussed at interview.

Our vision is ‘Working with all our partners to make a difference to the lives of people, through trusted, personalised and universal social care support, so people have choice and control, and can maximise their wellbeing and independence in their local community’.

Promoting independence, choice and control is at the heart of this position. This is an exciting opportunity to work with people to maximise their potential for independence.

You will have frequent direct contact with the public on the phone as well as visiting them in their own home or care setting. Your role will be to assist qualified practitioners with a variety of tasks, including assessing people’s needs and carrying out regular reviews. You will frequently be the first point of contact between the public and the service, gathering initial information and identifying the need for further assessment, as well as signposting people to other services.

You will work in partnership with individuals, their carers, professional staff in other agencies, service providers and Support Brokers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.

Social Care Assistants will hold a caseload of individuals and carers who will generally have a support plan in place that requires routine monitoring. Applicants will enjoy working with people and have experience of direct work with the public as part of paid or voluntary work, although they may have limited or no experience of visiting people in their own homes or care settings.

You will have the opportunity to work on the new ‘Discharge to Assess’ model to support vulnerable residents leaving hospital settings in a timely and safe manner. This exciting work will include working within hospitals as part of an integrated workforce of Health and Social Care practitioners with the aim of supporting our residents in their recovery in the community.

The Discharge to Assess model also provides continuity of care whereby, you as a practitioner, will see the resident’s progress from an acute hospital to the community, and provide Adult Social Care support to enable and empower residents to lead their fullest life whilst promoting their independence and decision-making.

You will need to have competent basic IT skills and be able and willing to learn to use a variety of software systems. The work involves travelling within Surrey; therefore, you will need to be a car driver and have access to a car.

As part of our greener future strategies and our commitment to creating a healthy work-life balance, we welcome a conversation with you regarding our remote working options. Remote working to us is a balance of spending time in our communities, our offices and working from home or wherever works best.
