Our client is the UK's leading dementia charity. They provide information and support, improve care, fund research, and create lasting change for people affected by dementia.

About the role.

Delivering one to one and group services out in the community (predominantly working from home until Coronavirus restrictions lift).

One-to-one Service support:

• Providing knowledgeable, compassionate and trusted advice and support to people affected by dementia.
• Helping people understand dementia and how to cope with day to day living whilst preparing for the future.
• Offering support to your clients whether this is in the client's own home, in the community, face to face, phone, letter or email.
• Assisting service users to access other services, providing signposting and referrals.
• Empowering individuals living with dementia to maintain their independence, improving their sense of wellbeing, and putting them in more control of their lives, making informed choices about their own life.
• Networking and building meaningful relationships with a wide range of local organisations, promoting services to help people with a diagnosis to get referred to the service as early as possible.
