If yes, our client is looking to welcome an approachable, enthusiastic, and knowledgeable, community-based Dementia Adviser who will focus on supporting people affected by Dementia across the South Hams, South Devon Area as part of the Devon and Torbay Dementia Adviser Service.
This is an exciting and rewarding opportunity providing support and guidance to people living with dementia and their carers. They strive to help people to maintain independence, improve their sense of well-being, and putting people in more control of their own lives. They promote a person-centred service so that their support is unique to each individual, based upon their own aspirations and needs.
About you
Trusted Experts – you will be joining an experienced team of Dementia Advisers across the South Devon Area, using a person-centred approach to provide expert local guidance, information and advice to people living with dementia and their family/carers.
Better Together they pride themselves on supporting people with an inclusive attitude and mindset is important to ensure they support everyone in the best way possible. There is an emphasis on partnership working and networking with Health and Social Care professionals, including the Older People’s Mental Health Team, GP surgeries, Social Prescribers, Devon Memory Service and Social Services. You will need experience in networking effectively and collaborating with other professionals to achieve a positive outcome for service users.

Compassionate – you will have a caring empathetic approach to supporting and empowering people affected by dementia.
