This role requires reasonable access to the Rotherham Metropolitan Borough area and requires regular travel around this area to groups, home visits and for networking.

About the role

Our client has a fantastic new opportunity for a Dementia Support Worker to join their skilled and supportive team! This role will take the lead in the delivery of their Social Prescribing specialist dementia enabling service which is commissioned by Voluntary Action Rotherham. This service is focused on supporting people to engage with their community, to improve confidence in accessing services, supporting people to stay independent and putting them more in control of their lives .

The role is community based and will involve working from home, accessing groups in the community and visiting people with dementia and their supporters in their home when needed. The role requires you to support people with dementia on a one to one basis looking at strategies and services which will support them to make connections in their community in a sustainable way.

This role will require you to:

  • Build relationships with a range of local contacts.
  • Network with health and care professionals, marketing our client's services to help people with a diagnosis to get referred to the service as early as possible.
  • Develop networks with partner organisations to promote the aim of empowering individuals living with dementia to make informed choices.
