Description

About the Role

Sutton Learning Disability Service supports people who are over the age of 18, live in Sutton and have been formally assessed to have a learning disability.

We currently have 2 Team Manager vacancies - one in our social work team for young people aged of 18-25 and one in our social work team for people who are 25 and older. Both teams work closely with our learning disability Clinical Health Team.

This is an exciting time to be joining Sutton, as we co-produce our new Learning Disability Strategy, in partnership with services users, families and other stakeholders. Key priorities include reducing health inequality and maximising opportunities for people with a learning disability to access work, independence enablement and self directed support opportunities.

We have also recently launched our Transition Protocol which will ensure a much smoother and more integrated progression for young people who will continue to require health and social care services as adults.
