Description
To provide short term intervention and support to service users and their families, and to lead a team of Reablement Support Workers. To ensure the smooth running of Reablement service delivery plans, supporting both the service users and the carers. To promote and encourage service users to be independent and to reconnect with their community. To increase social inclusion and skill development, to enhance quality of life.
The post holder is required to ensure compliance with relevant legislation and registration requirements as well as the approved policies and procedures of Powys County Council Social Care Department.
