Description
The Case Manager will manage a caseload of Prisoners in prison, supporting them to break down barriers to successful resettlement, achieve agreed milestones and enter employment, training or education. The Case Manager will provide initial assessments and undertake action planning to prisoners being released from in-scope prisons. Provide advice, guidance, coaching, mentoring and support to a caseload of participants via regular one-to-one meetings and group interventions as appropriate.
Case Manager
The Case Manager will manage a caseload of Prisoners in prison, supporting them to break down barriers to successful resettlement, achieve agreed milestones and enter employment, training or education. The Case Manager will provide initial assessments and undertake action planning to prisoners being released from in-scope prisons. Provide advice, guidance, coaching, mentoring and support to a caseload of participants via regular one-to-one meetings and group interventions as appropriate.
Pact’s Case Managers support Prisoners in custody to prepare for release and successfully resettle into their local communities. They will support the delivery of the National Offender Management Service (NOMS) CFO Programme providing practical help, advice and guidance, as well as motivation and understanding. Providing support for individuals with multiple and complex needs to overcome barriers and progress towards education, training and employment.
About You:
To be successful in this role, you will have the ability to multi-task, as well as experience and understanding of working with participants with complex multiple needs in a challenging multi-agency environment. You will have the ability to deliver rehabilitative interventions and groupwork as part of the CFO3 Contract within the West Midlands prisons and knowledge of safeguarding/child protection practice . You will also have excellent organisational skills and the ability to independently manage a demanding work load. You will have the ability to contribute to planning and development of Pact’s interventions. Furthermore you will have excellent interpersonal communication skills and ability to work in build partnerships with a range of agencies.

Hours: 37.5 hours per week, Monday to Friday (weekend & evening work may be required occasionally)
Salary: £24,000 per annum (£23,000 basic salary + £1,000 Market Supplement)
