Practitioner manager positions are now available in Lambeth Adult Social Care teams based at Guys and St Thomas Hospital and Kings College Hospital which are two of the UK’s leading NHS foundation trusts. This is a real opportunity to be a part of a services working closely as a system in partnership across Health and Social Care. Our goals is to facilitate safe and timely transfers of care from hospital to community and improve services and outcomes for our residents.

As a Practitioner Manager you will be responsible for management of front-line operational services which includes the provision a comprehensive assessment and case management service to vulnerable adults. Safeguarding Adults is one of our key focus areas and we expect all Lambeth staff to be committed to safeguarding and promoting the welfare of children and young people and vulnerable adults.

Health and Social Care are transforming and progressing toward more integrated working. There are a number of new initiatives which really makes our workplace dynamic and challenging. We need Practitioner Managers who can adapt and participate in change, innovate and seek continuous self-improvement and improvement of the services they deliver.

We are keen to enhance autonomy and our way of working provides user-led services that promote independence, choice and control. It gives you, and service users, an opportunity to influence service innovation and design with ‘personalisation’ being the cornerstone of our partnership with citizens. About the roles

We are looking for Practitioner Managers with up-to-date professional expertise, experience in complex case management, support planning and safeguarding.

We’ll support all Practitioner Managers fully with professional development opportunities so that you can build the experience you need for the career path you want.

We have a good track record of ‘growing our own’ and offer training to those interested in becoming a Practice Educator, Best Interest Assessor, or Approved Mental Health Practitioner as well as the opportunity to progress to other management roles. Staff surveys report high levels of satisfaction in the department. Feedback from recent staff surveys includes:
