Candidates should have a recognised social work qualification and registration with Social Care Wales.

The role involves managing a varied caseload, building on your skills and promoting your development alongside individuals, carers and staff. The post will give you the opportunity to consolidate your learning by completing assessments and care and support plans, Mental Capacity and Best Interests assessments, CHC applications, Court of Protection applications and Court work. Occasionally there will be a need to organise non-criminal investigations for Adults at Risk.

There will be the opportunity to gain training and support is provided by regular supervision and peer group sessions.

The role will involve supporting and supervising staff within the Adult and Transition Teams as well as coaching and mentoring.

We work as part of a Multi-Disciplinary Community Learning Disabilities Team and are fortunate to have our health colleagues sited with us for joint working, support and advice. The role involves working with service users, parents and carers to ensure the best outcomes for all and we strive to deliver the best possible service to people and promote their wellbeing.

Applicants will need to have skills in assessment of need and risk management and be able to effectively communicate with adults with learning disabilities, carers and providers of care services. Experience of working with individuals who have a learning disability would be preferred, although not essential, as long as the professional skills attained are transferable.

You will be working in a dynamic team who regularly work with some challenging and difficult situations. We expect that all staff will support each other through these times to obtain positive outcomes for all involved.

Presently we mainly work from home but a flexible approach to home office working is in place as is home visits with suitable risk assessments
