Description
Applications are invited for the post of Clinical Facilitator to work within the Clinical Effectiveness Group (CEG) in the Centre for Primary Care. CEG is an innovative team of healthcare professionals which aims to reduce illness and death through the promotion of evidence based health care of chronic disease in East London.
This is a highly visible post in which the clinical facilitator will be expected to be able to promote changes in behaviour and explain complex processes in a simple and clear manner and to build a positive perception of the CEG in the local health sector.
The postholder will have specific responsibilities around long term conditions such as diabetes and COPD and contribute to a number of local programmes across north east London.
The clinical facilitator will also have responsibility for developing clinical computer templates and training primary care staff in their use.
The successful candidate must be confident in communicating with both clinical and non-clinical staff at all levels across primary care. The candidate will need to have a very good knowledge of IT systems in primary care as this underpins all our programmes, and will require development and dissemination of clinical templates and other support tools within primary clinical systems.
Main duties and responsibilities
- To contribute and support the implementation of CEG work programmes related to Long Term Conditions(LTC)
- To facilitate GPs in use of clinical templates, establish recall systems & review patients records
- To work with clinical commissioners and providers to understand local clinical pathways & services
- To support GP practices in achieving targets within both local and national LTC programmes
- To develop or contribute to the development of clinical system tools such as templates, searches and protocols
- To provide reports and updates as needed to commissioning groups and boards on CEG programmes
- To lead on or support educational workshops
- To undertake such other duties as may be reasonably expected by the line manager or Head of Centre
