Description
Central is currently recruiting for two Support Workers, to work in the Blackheath area. The service provides support to young people aged 16-21 with medium-high support needs. The service provides 24/7 support and aims to empower independent living and provide housing support.
The successful candidate will be required to work 37.5 hours per week, working from Monday - Sunday on a rota basis, covering early (9AM - 4:30PM), mid (9AM - 6:30PM) and late (11:30AM - 8:30PM) shifts. You will be required to work two weekends per month. Cover is required for approximately 3 months. The pay rate is £12.43 per hour, with £1.50 holiday pay, totalling £13.93 per hour.
Please note, this is a Floating Support role and involves visiting services in the community.
In this position, you will be responsible for:
- Holding a caseload of 9 - 10 key clients and carrying out regular key working sessions resulting in agreed SMART Support Plans which are reviewed regularly
- Completing comprehensive and on-going needs and risk assessments
- Ensure that safeguarding and addressing the immediate needs of the young person are paramount to service delivery
- In conjunction with colleagues interview and comprehensively assess referrals to the project to ensure the criteria is met and suitable young people are accepted
- Provide a safe, welcoming, good standard of accommodation. Once referrals are accepted to ensure new residents are made to feel comfortable and are given information concerning the building and other local services
- Work with mediation or other social workers to enable young people to move back home if appropriate.
- Support young people to identify opportunities for move-on to appropriate PRS, other supported, shared or self-contained accommodation and make appropriate referrals
- Work with young people to identify activities that they would like to take part in, to prepare them for independence and move through
- Support young people that are ready to move into work, education or training by assisting them to access suitable courses or placements
- Work with young people to maximise benefit entitlements and assist young people for eventual move on into independent accommodation.
To apply for this role, you must have:
- Experience of working with at-risk young adults in a residential setting and ability to demonstrate through practice how 'Every Child Matters' relates to the young people living in our accommodation
- Experience of applying effective ways of working with young people, in particular; YP's with mental health and attachment needs including emerging personality disorder and/or significant self harm
- Experience of working in a recovery focused way and a good understanding of the principles and how they can be related to risk and needs assessment, planning, goal-setting and reviewing with young people
- Experience of working with young people to develop life skills and support their involvement in meaningful activities.
- Knowledge of welfare benefits for under 21's, rents and service charges.
