Lambeth is a fantastic place to work. If you’re looking for a challenging but exceptionally rewarding opportunity – Lambeth is definitely for YOU!
Lambeth is one of the most diverse and vibrant areas of London. Lambeth’s communities include Waterloo in the north, an area that has a world class arts and entertainment centre on the South Bank and a buzzing nightlife scene at Vauxhall. In the south we have Streatham’s bustling High Road and a wealth of open spaces and in Lambeth’s heart Brixton is well known for its vibrant, multicultural communities, bustling markets and the creative energy of its residents.
As part of Lambeth Together we are integrating Adult Social Care and NHS services to create new and more integrated ways of working. Our aim is to develop streamlined multidisciplinary services that focus on individuals and help build stronger communities.
We are currently recruiting Assessors for the following teams:
- Information, Contact and Assessment Service (front door team)
- Community Duty Team
Information, Contact & Assessment Service (iCAS) is the front door team for Adult Social Care. It undertakes Care Act Assessments for all clients who are new to the service and completes safeguarding concerns. It also screens OT referrals and undertakes preventative intervention work including provision of minor equipment. The team works in partnership with Age UK Lambeth who screen new referrals into their services or refers onto other voluntary sector organisations while it triages all referrals that require a statutory service to iCAS.
Community Duty Team is a duty function which provides a centralised point for known service users who are not allocated but receive a service. The team work generically across Physical Disabilities, Older Persons, and Learning Disabilities cases to resolve immediate issues for those not in need of a review or resolve immediate issues for cases where the allocated worker is not available.
We are looking for dynamic and curious individual who is able to identify people’s strengths, offering support to enable individuals to find solutions.
This is an excellent opportunity if you share our commitment to promoting independence and supporting people to find solutions to maintain their well-being.
As an Assessor you should have the ability to work at a fast pace and to gather relevant information from individuals to enable you to determine the next course of action required. You should have an understanding of risk and care planning, the ability to identify potential services and evaluate the costs/benefits of different options as well as develop new initiatives and negotiate with service providers across the sector, we aim to develop strong links with our local partners to ensure we can offer creative support options that maximise a person’s self – determination and independence.
Assessors within Lambeth are provided with training to apply detailed knowledge of available basic, standard equipment (e.g. rails) and decide on the best options for the provision of specialist equipment, minor adaptations and assistive technology: in consultation with management deciding between issuing prescriptions, equipment delivery service, contractors, referral to an external agency or sign post to local retailers. Additionally, there is the opportunity to develop strong social care assessment skills.
